Roles & Responsibilities

• Provide compassionate, patient-centered nursing care as an LPN on the unit
• Administer medications and document accurately and timely
• Monitor vital signs and recognize changes in patient condition promptly
• Perform treatments and wound care according to care plans and protocols
• Collaborate with RNs, providers, and the care team to deliver safe care
• Communicate clearly with patients and families to educate and support them
• Maintain accurate electronic health record documentation and handoffs

Qualifications

• Valid LPN license in Pennsylvania (required)
• Current BLS/CPR certification (or willingness to obtain upon hire)
• Prior clinical experience as an LPN preferred, especially in long-term care or med-surg
• Ability to pass facility background checks and required screenings
• Strong interpersonal skills and a patient-focused attitude

Education

• Completion of an accredited LPN/vocational nursing program

Knowledge & Skills

• Proven skills in medication administration and basic nursing procedures
• Comfortable with vital sign assessment, wound care, and ADL support
• Familiarity with electronic health records and accurate charting
• Ability to prioritize tasks in a fast-paced care environment
• Good clinical judgment and effective communication with patients and team members
