Roles & Responsibilities

• Provide compassionate, hands-on nursing care to residents in accordance with facility policies
• Administer medications and treatments accurately and document care clearly
• Assess patient status, monitor vital signs, and report changes to the care team
• Collaborate with nurses, therapists, and physicians to deliver coordinated care
• Support residents with activities of daily living while promoting dignity and independence
• Participate in care planning, accurate charting, and handoffs for safe continuity

Qualifications

• Current Licensed Practical Nurse (LPN) license — active and in good standing (PA license recommended)
• Current Basic Life Support (BLS) / CPR certification or willingness to obtain
• Ability to pass required background checks and facility onboarding
• Comfortable working in a skilled nursing or long-term care environment; previous LPN experience preferred
• Strong communication and teamwork skills; patient-focused and dependable

Education

• Completion of an accredited LPN / Practical Nursing program; high school diploma or GED required

Knowledge & Skills

• Clinical skills in medication administration, wound care, and basic assessments
• Familiarity with electronic charting and accurate documentation practices
• Infection control principles and safe patient handling techniques
• Time management and prioritization in a busy care setting
• Compassionate bedside manner and strong interpersonal communication
