Contract Type: Perm
Shift Type: Monday-Friday, 8:00 AM - 4:30 PM (Day), 32 hours/week, no weekend rotation
Hours per Week: 43
Candidate Pay Rate: $40 per hour

Roles & Responsibilities
- Assess patients to determine physical therapy needs and goals
- Develop and implement individualized treatment plans
- Provide hands-on therapy and therapeutic exercise instruction
- Document patient progress and maintain accurate clinical records
- Collaborate with interdisciplinary team members to optimize outcomes
- Educate patients and families on home programs and injury prevention
- Participate in quality improvement initiatives and maintain clinical standards
Qualifications
- Licensed Physical Therapist in Illinois or eligible for Illinois licensure
- Experience or training in physical therapy practice (outpatient/rehab experience a plus)
- Current BLS/CPR certification or willingness to obtain
- Strong clinical reasoning, documentation skills, and patient communication
- Ability to work a consistent Monday–Friday daytime schedule
Education
- Doctor of Physical Therapy (DPT) or Master's in Physical Therapy preferred
Knowledge & Skills
- Knowledge of therapeutic exercise, manual therapy, and rehabilitation techniques
- Skilled in patient assessment, goal-setting, and progress evaluation
- Proficient with clinical documentation and electronic health records
- Strong interpersonal skills and the ability to educate patients effectively
- Ability to collaborate with nurses, physicians, and other therapy staff
