Roles & Responsibilities

• Provide patient-centered physical therapy evaluations and develop individualized treatment plans
• Deliver skilled therapeutic interventions including exercise, manual therapy, and functional training
• Collaborate with nurses, physicians, and the interdisciplinary team to coordinate care
• Document assessments, treatment progress, and discharge plans in a timely and accurate manner
• Educate patients and families on home programs, safe mobility, and prevention strategies
• Participate in quality improvement initiatives and contribute to a positive facility culture
• Assist with safe patient transfers and equipment recommendations to maximize outcomes

Qualifications

• Current and unrestricted Pennsylvania Physical Therapist license required
• Graduate of an accredited Physical Therapy program; DPT preferred
• Basic Life Support (BLS/CPR) certification required or obtained upon hire
• Experience in rehabilitation settings such as inpatient, outpatient, or long-term care preferred
• Strong clinical reasoning, documentation skills, and patient communication

Education

• Doctor of Physical Therapy (DPT) preferred; Master or Bachelor in Physical Therapy considered

Knowledge & Skills

• Skilled in evaluation, treatment planning, and therapeutic exercise
• Experience with gait training, balance, and functional mobility interventions
• Familiarity with rehab documentation systems and regulatory requirements
• Excellent communication and teamwork skills
• Ability to safely lift, transfer, and assist patients as needed
