Roles & Responsibilities

• Provide compassionate direct patient care for assigned residents and clients
• Administer medications and treatments accurately and document in the chart
• Monitor and record vital signs, reporting changes to the care team promptly
• Collaborate with RNs and interdisciplinary staff to support individualized care plans
• Assist with admissions, discharges, and transfers while ensuring continuity of care
• Educate patients and families about care, medications, and follow-up needs
• Maintain a safe, clean environment and adhere to facility policies and infection control standards

Qualifications

• Current Licensed Practical Nurse (LPN) license — required
• Recent clinical experience preferred (experience in long-term care, skilled nursing, or rehab is a plus)
• Basic Life Support (BLS) certification preferred or willingness to obtain
• Strong communication skills and ability to work collaboratively on a care team
• Comfortable with electronic health record documentation

Education

• Completion of an accredited Practical Nursing program or equivalent vocational training

Knowledge & Skills

• Medication administration and accurate documentation
• Patient assessment and vital sign monitoring
• Wound care basics and assistance with ADLs
• Charting in electronic health records (EHR)
• Effective communication with patients, families, and clinical teams
• Time management and prioritization during busy shifts
