Roles & Responsibilities

• Provide compassionate hands-on nursing care to residents and patients
• Administer medications safely and document treatments accurately
• Perform routine assessments, monitor vital signs, and report changes promptly
• Assist with activities of daily living while promoting independence and dignity
• Collaborate with the interdisciplinary team to support individualized care plans
• Maintain accurate, timely documentation in the patient record
• Follow infection control practices and facility policies to ensure safety

Qualifications

• Active LPN license in Pennsylvania required
• Current BLS/CPR certification or willingness to obtain upon hire
• Completion of an accredited practical nursing program
• Ability to pass facility background check and drug screening
• Immunizations up to date per facility policy
• Strong communication skills and a team-first attitude

Education

• Diploma or Certificate from an accredited Practical Nursing program
• High school diploma or GED required

Knowledge & Skills

• Medication administration and documentation best practices
• Patient assessment and basic clinical procedures
• Reliable vitals monitoring and charting
• Comfortable using electronic health records and standard clinical forms
• Infection prevention and safe patient handling techniques
• Clear communication with patients, families, and the care team
• Time management and prioritization in a fast-paced environment
