Roles & Responsibilities

• Evaluate patients' functional abilities and create individualized treatment plans
• Provide skilled therapeutic interventions to restore mobility and independence
• Document progress thoroughly and communicate outcomes with the care team
• Collaborate with nurses, speech and occupational therapists, and physicians to optimize patient care
• Educate patients and families on safe mobility, exercise programs, and fall prevention
• Supervise and mentor therapy assistants and aides to ensure quality care
• Contribute to discharge planning to support successful transitions back to the community

Qualifications

• Licensed Physical Therapist in Pennsylvania or eligible for state licensure
• Graduate of an accredited Physical Therapy program (DPT preferred)
• Current BLS/CPR certification
• Experience in skilled nursing or post-acute care preferred
• Strong clinical reasoning, documentation skills, and ability to work as part of a multidisciplinary team

Education

• Doctor of Physical Therapy (DPT) from an accredited program preferred; Master's acceptable if applicable

Knowledge & Skills

• Proficient in assessment and treatment planning for geriatric and post-acute populations
• Knowledge of therapeutic modalities, therapeutic exercise, and ADL retraining
• Familiarity with assistive devices, transfers, gait training, and fall prevention strategies
• Comfortable with electronic medical records and timely documentation
• Excellent communication, time management, and teamwork skills
