Roles & Responsibilities
- Perform thorough occupational therapy evaluations and create individualized treatment plans
- Deliver evidence based interventions to improve clients' daily living and functional independence
- Collaborate with nurses, therapists and care teams to coordinate patient centered care
- Document progress accurately and timely in the patient record
- Educate patients and families on home programs and adaptive strategies to maximize outcomes
- Participate in care planning and discharge planning to ensure safe transitions
Qualifications
- Licensed or eligible for licensure as an Occupational Therapist in Michigan
- Graduation from an accredited occupational therapy program
- Current basic life support certification preferred
- Comfortable working with adult and geriatric populations in a rehabilitation setting
- Strong clinical reasoning, communication and documentation skills
- Team player attitude and commitment to patient centered care
Education
- Degree from an accredited Occupational Therapy program (Bachelor's, Master's or entry level clinical degree as required for licensure)
Knowledge & Skills
- Skilled in OT assessment and treatment planning
- Knowledge of therapeutic exercises, ADL retraining and assistive devices
- Experience with documentation and electronic health records
- Ability to collaborate across disciplines and communicate with families
- Time management and caseload prioritization for efficient care delivery
