Roles & Responsibilities

• Evaluate residents to develop individualized treatment plans that restore mobility and independence
• Deliver skilled therapeutic interventions including manual therapy, therapeutic exercise and gait training
• Document progress and functional outcomes clearly and timely in the medical record
• Collaborate with nurses, activities staff and physicians to coordinate care and meet patient goals
• Educate residents and families on home programs, fall prevention and safe mobility strategies
• Participate in team meetings and contribute to quality improvement initiatives in the rehab unit

Qualifications

• Valid California Physical Therapist license or eligible to obtain licensure in California
• Basic Life Support (BLS/CPR) certification or willingness to obtain on hire
• Experience in skilled nursing, inpatient rehab or post-acute care preferred
• Strong clinical reasoning, documentation skills, and ability to work collaboratively
• Clear background check and ability to meet facility onboarding requirements
• Excellent communication skills and a compassionate approach to patient care

Education

• Doctor of Physical Therapy (DPT) preferred or entry-level PT degree from an accredited program
• Graduate of an accredited physical therapy program and eligible for state licensure

Knowledge & Skills

• Skilled in patient assessment and creating individualized rehab plans
• Proficient with therapeutic modalities and manual therapy techniques
• Strong knowledge of mobility aids, transfer techniques and fall prevention
• Comfortable documenting outcomes and complying with regulatory standards
• Able to work well in interdisciplinary teams and mentor support staff
• Time management and caseload prioritization in a busy skilled nursing setting
