Shift Type: Monday-Friday, 8:00 AM - 4:30 PM (Day shift), 35 hours/week, No weekend rotation
Hours per Week: 43
Candidate Pay Rate: $45 per hour

Roles & Responsibilities
- Provide high-quality therapeutic interventions and assist patients with mobility, strengthening and functional activities
- Assist the supervising Physical Therapist in implementing individualized treatment plans
- Perform gait training, transfers and use of assistive devices to maximize patient independence
- Document patient progress and treatment notes clearly and timely in the medical record
- Communicate effectively with patients, families and interdisciplinary team members to support coordinated care
- Educate patients and caregivers on home exercise programs and safety strategies
- Participate in quality improvement initiatives and contribute to a positive facility environment
Qualifications
- Licensed or certificated as a Physical Therapist Assistant (PTA) — ability to meet state licensure requirements
- Current Basic Life Support (BLS/CPR) certification (or willing to obtain)
- Experience in post-acute, skilled nursing or rehabilitation settings preferred
- Strong communication skills and a patient-centered approach
- Ability to perform physical tasks associated with patient care and safely assist with transfers
Education
- Associate degree or diploma from an accredited Physical Therapist Assistant program
Knowledge & Skills
- Knowledge of therapeutic exercise and modalities commonly used in rehabilitation
- Proficiency with gait training, transfers and ADL-focused interventions
- Comfortable documenting in electronic medical records and following care plans
- Good clinical judgment, time management and organizational skills
- Ability to work collaboratively in an interdisciplinary team
