Roles & Responsibilities

• Provide high-quality therapeutic interventions and assist patients with mobility, strengthening and functional activities
• Assist the supervising Physical Therapist in implementing individualized treatment plans
• Perform gait training, transfers and use of assistive devices to maximize patient independence
• Document patient progress and treatment notes clearly and timely in the medical record
• Communicate effectively with patients, families and interdisciplinary team members to support coordinated care
• Educate patients and caregivers on home exercise programs and safety strategies
• Participate in quality improvement initiatives and contribute to a positive facility environment

Qualifications

• Licensed or certificated as a Physical Therapist Assistant (PTA) — ability to meet state licensure requirements
• Current Basic Life Support (BLS/CPR) certification (or willing to obtain)
• Experience in post-acute, skilled nursing or rehabilitation settings preferred
• Strong communication skills and a patient-centered approach
• Ability to perform physical tasks associated with patient care and safely assist with transfers

Education

• Associate degree or diploma from an accredited Physical Therapist Assistant program

Knowledge & Skills

• Knowledge of therapeutic exercise and modalities commonly used in rehabilitation
• Proficiency with gait training, transfers and ADL-focused interventions
• Comfortable documenting in electronic medical records and following care plans
• Good clinical judgment, time management and organizational skills
• Ability to work collaboratively in an interdisciplinary team
