Roles & Responsibilities

• Perform thorough assessments and develop individualized treatment plans to help patients regain function and independence
• Deliver skilled therapeutic interventions using evidence-based techniques and safe handling practices
• Document patient progress accurately and timely in the medical record to support continuity of care
• Collaborate with nurses, physicians and interdisciplinary team members to coordinate comprehensive care
• Educate patients and families on home exercise programs, fall prevention and strategies to maximize outcomes
• Monitor and evaluate treatment effectiveness, adjusting plans of care to meet patient goals
• Participate in team meetings and contribute to quality improvement initiatives to enhance patient experience

Qualifications

• Licensed Physical Therapist in California (or eligible for licensure)
• Current BLS/CPR certification (or willing to obtain upon hire)
• Experience in skilled nursing/rehabilitation settings preferred
• Strong clinical assessment and documentation skills
• Excellent communication and teamwork skills, with a patient-first attitude
• Ability to work a 35-hour schedule, including weekend rotation as needed

Education

• Doctor of Physical Therapy (DPT) preferred or Master's in Physical Therapy

Knowledge & Skills

• Skilled in functional mobility assessment and therapeutic exercise prescription
• Familiarity with documentation in electronic medical records
• Knowledge of safety standards, transfer techniques and fall prevention
• Ability to prioritize caseloads and manage time effectively
• Strong interpersonal skills for patient and family education
