Roles & Responsibilities

• Provide skilled physical therapy interventions under the direction of a licensed PT, focusing on restoring mobility and function
• Implement treatment plans such as therapeutic exercises, gait training, transfers, and functional activities
• Document patient progress and outcomes clearly and timely in the medical record
• Educate patients and families on home programs, safety, and mobility strategies
• Collaborate with the interdisciplinary care team to coordinate patient-centered care
• Monitor and maintain equipment and treatment areas to ensure safety and cleanliness
• Participate in team meetings and contribute to quality improvement and patient satisfaction efforts

Qualifications

• Licensed Physical Therapist Assistant (PTA) in Wisconsin or eligible for Wisconsin licensure
• Graduated from an accredited PTA program (Associate degree or equivalent)
• Current BLS/CPR certification (or willing to obtain)
• Experience in rehab, long-term care, or geriatrics is a plus
• Strong communication skills and a patient-centered approach

Education

• Associate Degree in Physical Therapist Assistant from an accredited program

Knowledge & Skills

• Skilled in therapeutic exercise, gait training, transfers, and functional mobility
• Knowledge of safety and infection control practices
• Familiarity with common rehab modalities and adaptive equipment
• Comfortable with documentation in electronic medical records
• Teamwork, time management, and clear patient education skills
