Candidate Pay Rate: $40 per hour

Roles & Responsibilities
- Provide thorough evaluations and create individualized physical therapy treatment plans
- Deliver hands-on skilled therapy and evidence-based interventions to help patients meet goals
- Collaborate closely with the interdisciplinary team to coordinate exceptional patient care
- Document patient progress and outcomes accurately and timely in the medical record
- Educate patients and caregivers on home programs, safety, and strategies for independence
- Participate in quality improvement initiatives to improve clinical outcomes and patient experience
Qualifications
- Doctor of Physical Therapy (DPT) or Master’s in Physical Therapy preferred
- Licensed or eligible for Physical Therapist licensure in Wisconsin
- Current BLS/CPR certification or willingness to obtain
- Strong communication and interpersonal skills; you connect with patients and the team
- Comfortable documenting in an electronic medical record and following facility protocols
- Previous experience in rehab or outpatient settings is a plus
Education
- Doctor of Physical Therapy (DPT) or Master’s degree in Physical Therapy
Knowledge & Skills
- Skilled in therapeutic exercise, manual therapy, and functional mobility training
- Ability to develop and progress individualized treatment plans
- Knowledge of outcome measures and documentation best practices
- Patient education and caregiver training to promote independence
- Good time management and caseload prioritization skills
