Roles & Responsibilities

• Provide high-quality, resident-centered physical therapy interventions under the direction of a supervising PT
• Assist with therapeutic exercises, gait training, transfers, and ambulation to improve mobility and independence
• Monitor and document patient progress accurately and promptly in the medical record
• Collaborate with the interdisciplinary care team to develop and adjust individualized care plans
• Educate residents and family members on exercise programs, safety techniques, and fall prevention
• Maintain a safe treatment environment and adhere to infection control and safety protocols
• Participate in team meetings and contribute to a positive, resident-focused culture

Qualifications

• Licensed Physical Therapist Assistant (PTA) in Oklahoma or ability to obtain state licensure
• Graduate of an accredited PTA program
• Current CPR/BLS certification or willingness to obtain
• Comfortable working in a continuing care/CCRC setting; previous experience preferred
• Strong communication and documentation skills with attention to detail
• Ability to pass background check and meet facility onboarding requirements

Education

• Associate Degree from an accredited Physical Therapist Assistant program

Knowledge & Skills

• Skilled in therapeutic exercise, mobility training, transfers, and use of adaptive equipment
• Familiarity with geriatric care and long-term care rehabilitation needs
• Proficient documentation and charting practices, comfortable with electronic medical records
• Strong interpersonal skills and the ability to work well within a multidisciplinary team
• Good clinical judgment, time management, and resident-centered problem solving
