Roles & Responsibilities
- Provide high-quality, resident-centered physical therapy interventions under the direction of a supervising PT
- Assist with therapeutic exercises, gait training, transfers, and ambulation to improve mobility and independence
- Monitor and document patient progress accurately and promptly in the medical record
- Collaborate with the interdisciplinary care team to develop and adjust individualized care plans
- Educate residents and family members on exercise programs, safety techniques, and fall prevention
- Maintain a safe treatment environment and adhere to infection control and safety protocols
- Participate in team meetings and contribute to a positive, resident-focused culture
Qualifications
- Licensed Physical Therapist Assistant (PTA) in Oklahoma or ability to obtain state licensure
- Graduate of an accredited PTA program
- Current CPR/BLS certification or willingness to obtain
- Comfortable working in a continuing care/CCRC setting; previous experience preferred
- Strong communication and documentation skills with attention to detail
- Ability to pass background check and meet facility onboarding requirements
Education
- Associate Degree from an accredited Physical Therapist Assistant program
Knowledge & Skills
- Skilled in therapeutic exercise, mobility training, transfers, and use of adaptive equipment
- Familiarity with geriatric care and long-term care rehabilitation needs
- Proficient documentation and charting practices, comfortable with electronic medical records
- Strong interpersonal skills and the ability to work well within a multidisciplinary team
- Good clinical judgment, time management, and resident-centered problem solving
