Roles & Responsibilities

• Evaluate patients and perform thorough assessments to create individualized treatment plans
• Implement evidence-based physical therapy interventions to improve mobility and function
• Document patient progress clearly and timely to support continuity of care
• Collaborate with nursing, rehabilitation team members, and families to optimize outcomes
• Educate patients and caregivers on home programs, safety, and fall prevention
• Participate in team meetings and contribute to a positive, patient-centered culture

Qualifications

• Valid Pennsylvania Physical Therapist license (required)
• Current BLS/CPR certification (preferred or obtain upon hire)
• Experience in skilled nursing, outpatient, or rehabilitation settings preferred
• Strong clinical reasoning and patient communication skills
• Ability to work the posted daytime schedule and engage with interdisciplinary teams

Education

• Doctor of Physical Therapy (DPT) preferred or equivalent degree in Physical Therapy
• Graduation from an accredited physical therapy program

Knowledge & Skills

• Skilled in therapeutic exercise, manual therapy techniques, and mobility training
• Knowledge of documentation best practices and therapeutic goal setting
• Ability to assess and manage patients with mobility, balance, and strength deficits
• Comfortable using electronic medical records and standard rehab equipment
• Excellent interpersonal skills and a patient-centered approach
