Roles & Responsibilities

• Evaluate patients’ physical function, mobility and pain to build meaningful plan of care
• Develop and implement individualized treatment plans using evidence-based interventions
• Provide hands-on therapy, therapeutic exercise, and functional training to improve outcomes
• Monitor progress and modify treatment plans to ensure measurable patient improvement
• Educate patients and caregivers on home exercise programs and fall prevention strategies
• Collaborate with interdisciplinary team members to coordinate comprehensive care
• Document assessments, interventions and outcomes accurately and timely

Qualifications

• Valid Pennsylvania Physical Therapist license (required)
• Doctor of Physical Therapy (DPT) or Master’s in Physical Therapy preferred
• Current CPR/BLS certification (or willing to obtain)
• Clinical experience delivering skilled physical therapy in post-acute or outpatient settings preferred
• Strong communication skills and a patient-centered approach

Education

• DPT (Doctor of Physical Therapy) preferred; Master’s in Physical Therapy accepted

Knowledge & Skills

• Skilled in therapeutic exercise, neuromuscular re-education and manual therapy techniques
• Able to perform thorough functional assessments and write clear, measurable goals
• Comfortable using outcome measures and documenting progress in electronic medical records
• Excellent interpersonal skills for coaching patients and collaborating with care teams
• Time management and caseload planning to deliver high-quality care efficiently
