Contract Type: Perm
Shift Type: Rotating day shifts: 5 days/week, 8:00 AM - 4:30 PM with weekend rotation (day shift)
Hours per Week: 43
Candidate Pay Rate: $44 per hour

Roles & Responsibilities
- Provide patient-centered physical therapy evaluations and develop individualized treatment plans
- Deliver skilled treatment interventions to improve mobility, strength, and functional independence
- Collaborate with nurses, therapists, and family members to coordinate safe, effective care
- Document patient progress and revise plans of care to reflect measurable outcomes
- Educate patients and caregivers on home exercise programs and safe mobility strategies
- Participate in interdisciplinary team meetings to advocate for patient goals and transitions of care
Qualifications
- Licensed Physical Therapist in Pennsylvania (or eligible for licensure)
- Active professional liability insurance (or willing to obtain)
- Comfortable working in a skilled nursing/rehab environment and participating in weekend rotation
- Strong clinical assessment and manual therapy skills with good documentation practices
- Excellent communication skills and a collaborative mindset
Education
- Degree in Physical Therapy (BPT/MPT) or Doctor of Physical Therapy (DPT) preferred
Knowledge & Skills
- Skilled in physical therapy evaluation and treatment planning
- Knowledge of rehab protocols for mobility, strength, balance, and ADL training
- Familiarity with documentation best practices and outcomes measurement
- Ability to prioritize caseload and work effectively within an interdisciplinary team
- Patient education and caregiver training expertise
