Roles & Responsibilities

• Provide skilled physical therapy evaluations and design individualized treatment plans that help patients regain mobility and independence
• Deliver hands-on and therapeutic interventions including gait training, therapeutic exercise, and functional mobility training
• Monitor patient progress, adjust treatment plans, and document outcomes clearly and promptly
• Collaborate with nurses, physicians, and the interdisciplinary team to coordinate care and optimize recovery
• Educate patients and families on home exercise programs and safe functional strategies for daily living
• Maintain accurate clinical documentation in the medical record and participate in quality improvement initiatives
• Support a safe, patient-centered environment and advocate for best practices in rehabilitation

Qualifications

• Licensed Physical Therapist in Colorado (required)
• Doctor of Physical Therapy (DPT) preferred; Master's in Physical Therapy or equivalent accepted
• Current BLS/CPR certification or willingness to obtain
• Experience in inpatient rehabilitation, sub-acute, or similar settings preferred
• Strong communication, teamwork, and clinical decision-making skills
• Ability to perform patient transfers and mobility tasks safely

Education

• Doctor of Physical Therapy (DPT) preferred; Master's in Physical Therapy accepted

Knowledge & Skills

• Skilled in therapeutic exercise prescription and functional mobility training
• Experience with gait training, balance interventions, and assistive device recommendations
• Familiarity with manual therapy techniques and common rehab modalities
• Proficient clinical documentation and outcomes measurement
• Comfortable working within an interdisciplinary team and educating patients/families
• Basic proficiency with electronic health records and charting standards
