Roles & Responsibilities

• Conduct skilled occupational therapy evaluations and develop individualized treatment plans
• Deliver evidence-based therapeutic interventions to support functional independence
• Document patient progress accurately and promptly in the medical record
• Collaborate with nurses, PT, SLP and the interdisciplinary team to coordinate care
• Educate patients and families on home programs and adaptive strategies
• Monitor and adjust therapy goals to ensure meaningful outcomes and safe discharges
• Participate in team meetings and contribute to quality improvement initiatives

Qualifications

• Current Occupational Therapist license in Texas or eligibility to obtain state licensure
• Graduation from an accredited Occupational Therapy program (Master's or OTD preferred)
• Previous experience in rehabilitation, skilled nursing, or outpatient settings preferred
• Strong documentation skills and familiarity with therapy charting standards
• Excellent communication and teamwork skills with a patient-centered approach
• Basic Life Support (BLS) certification preferred or willing to obtain

Education

• Master's degree in Occupational Therapy or entry-level Doctorate in Occupational Therapy (OT/OTD) from an accredited program

Knowledge & Skills

• Skilled in OT assessment and treatment planning for adult/geriatric populations
• Knowledge of adaptive equipment, ADL retraining, and therapeutic modalities
• Proficient clinical reasoning and outcome-driven intervention strategies
• Familiarity with electronic medical records and accurate clinical documentation
• Ability to collaborate effectively within an interdisciplinary team
