Shift Type: Monday-Friday, 8:00 AM - 4:30 PM (Day), 35 hours/week

Candidate Pay Rate: $41 per hour

Roles & Responsibilities
- Assess patients' physical function and mobility to create individualized treatment plans
- Deliver skilled therapeutic interventions to improve strength, balance, and independence
- Educate patients and families on home exercise programs and safe mobility techniques
- Collaborate with nurses, physicians, and therapists to coordinate comprehensive care
- Document patient progress thoroughly and timely in the medical record
- Monitor outcomes and adjust treatment plans to maximize patient recovery
- Support discharge planning and provide recommendations for continued care
Qualifications
- Licensed Physical Therapist in Illinois or eligible for licensure
- Degree in Physical Therapy (DPT, MPT, or equivalent)
- Current BLS/CPR certification or willingness to obtain upon hire
- Strong clinical assessment and manual therapy skills
- Excellent communication and teamwork abilities
- Ability to work the posted schedule (Monday-Friday, daytime)
Education
- Doctor of Physical Therapy (DPT) preferred; MPT or equivalent accepted
Knowledge & Skills
- Skilled in therapeutic exercise, mobility training, and gait training
- Proficient with documentation and basic electronic health record systems
- Knowledge of safety principles for patient handling and fall prevention
- Ability to set measurable goals and track functional outcomes
- Comfortable working in a fast-paced rehabilitation environment
