Contract Type: Perm
Shift Type: Monday-Friday, 8:00 AM - 4:30 PM (Day) — 35 hours/week, no weekend rotation
Hours per Week: 35
Candidate Pay Rate: $40 per hour

Roles & Responsibilities
- Evaluate and treat patients using evidence-based physical therapy interventions to maximize function and independence
- Develop and implement individualized treatment plans in collaboration with patients and families
- Work closely with skilled nursing staff and the interdisciplinary team to coordinate care and improve outcomes
- Document patient progress thoroughly and communicate changes in condition to the care team
- Educate patients and caregivers on home exercise programs, safety, and long-term recovery strategies
- Participate in quality improvement initiatives to enhance patient care and facility performance
Qualifications
- Current Physical Therapist license in Wisconsin or eligibility to obtain one
- Doctor of Physical Therapy (DPT) or equivalent clinical degree preferred
- Experience in skilled nursing or post-acute care is a plus
- Basic Life Support (BLS) certification preferred or willing to obtain
- Strong clinical reasoning, communication, and teamwork skills
Education
- Doctor of Physical Therapy (DPT) preferred; Master’s or Bachelor’s in Physical Therapy or equivalent considered
Knowledge & Skills
- Skilled in therapeutic exercise, manual therapy, and functional mobility training
- Ability to assess gait, balance, and ADL function and create targeted interventions
- Familiarity with documentation systems and regulatory compliance in post-acute/skilled nursing settings
- Excellent interpersonal skills for patient and family education
- Comfortable collaborating with nursing, rehab assistants, and interdisciplinary teams
