Roles & Responsibilities

• Provide high-quality physical therapy evaluation and treatment to adult and/or geriatric patients
• Develop individualized treatment plans that restore function and improve quality of life
• Use evidence-based therapeutic techniques and modalities to achieve measurable outcomes
• Document patient progress accurately and communicate updates with the interdisciplinary team
• Educate patients and families on home programs and prevention strategies
• Collaborate with nursing and rehabilitation staff to ensure coordinated, person-centered care

Qualifications

• Active Pennsylvania Physical Therapist license or eligible for licensure in PA
• Doctor of Physical Therapy (DPT) or Bachelor’s degree in Physical Therapy from an accredited program
• BLS/CPR certification (or willingness to obtain upon hire)
• Comfortable with documentation and common rehab software
• Strong clinical assessment, manual therapy, and therapeutic exercise skills
• Excellent communication and teamwork skills; prior skilled nursing or rehab experience is a plus

Education

• Doctor of Physical Therapy (DPT) preferred; Bachelor’s in Physical Therapy accepted

Knowledge & Skills

• Skilled in patient evaluation, treatment planning, and therapeutic exercise
• Knowledge of rehabilitation techniques for skilled nursing and long-term care populations
• Ability to safely mobilize and transfer patients
• Proficient documentation practices and outcome measurement
• Strong interpersonal skills for patient and family education
• Familiarity with infection control and facility policies
