Roles & Responsibilities

• Assess patients and evaluate mobility, strength, balance, and functional needs to create meaningful goals
• Develop and implement individualized treatment plans using evidence-based interventions
• Provide skilled therapeutic techniques including therapeutic exercise, gait training, and manual therapy
• Document patient progress accurately and promptly in the electronic health record
• Collaborate with nurses, physicians, and other therapists to coordinate safe, effective care
• Educate patients and families on home programs, safety, and strategies to maximize independence
• Maintain therapy equipment and ensure a safe treatment environment
• Participate in quality improvement initiatives to enhance patient outcomes

Qualifications

• Licensed Physical Therapist in New Hampshire (or eligible for licensure)
• Current BLS/CPR certification (or willingness to obtain)
• Experience in inpatient rehab, skilled nursing, or similar settings preferred
• Strong clinical assessment, treatment planning, and documentation skills
• Excellent communication skills and a collaborative mindset
• Ability to handle a caseload and prioritize patient needs effectively

Education

• Doctor of Physical Therapy (DPT) preferred; Bachelor's in Physical Therapy considered

Knowledge & Skills

• Patient assessment and development of individualized treatment plans
• Therapeutic exercise, manual therapy, and functional mobility training
• Gait training, transfers, and use of assistive devices
• Familiarity with common rehab equipment and adaptive devices
• Proficient documentation skills in EHR systems
• Knowledge of safety, infection control, and fall prevention
• Interdisciplinary teamwork and patient/family education
