Contract Type: Perm
Shift Type: Monday-Friday, 8:00 AM - 4:30 PM Day shift (35 hours/week; weekends as needed)

Candidate Pay Rate: $40 per hour

Roles & Responsibilities
- Assess patients’ mobility, strength, balance and functional needs and create individualized treatment plans
- Deliver evidence-based therapeutic interventions to restore movement and reduce pain
- Monitor and document patient progress clearly and communicate changes to the care team
- Educate patients and families on home exercise programs and safe mobility techniques
- Collaborate with interdisciplinary staff to coordinate patient-centered rehabilitation care
- Participate in discharge planning and provide recommendations to optimize long-term outcomes
Qualifications
- Current Physical Therapist license in New Hampshire or eligible for licensure
- Doctor of Physical Therapy (DPT) or Bachelor/Master in Physical Therapy and relevant clinical experience
- Strong clinical assessment and therapeutic intervention skills
- Excellent communication and teamwork abilities
- CPR/BLS certification (or willingness to obtain upon hire)
- Background check and any facility-required clearances
Education
- Doctor of Physical Therapy (DPT) preferred; or Bachelor/Master in Physical Therapy and required state licensure
Knowledge & Skills
- Skilled in musculoskeletal and functional rehabilitation techniques
- Knowledge of therapeutic modalities, exercise prescription and gait training
- Proficient documentation and outcome measurement practices
- Ability to adapt care plans to varied patient needs and acuity levels
- Comfortable working as part of an interdisciplinary rehabilitation team
