Contract Type: Perm
Shift Type: Tuesday-Saturday, 8:00 AM - 4:30 PM (Day) or Sunday-Thursday, 8:00 AM - 4:30 PM (Day); 35 hours/week; No weekend rotation
Hours per Week: 43
Candidate Pay Rate: $37 per hour

Roles & Responsibilities
- Evaluate patients to assess functional abilities and identify meaningful therapy goals
- Develop and implement individualized occupational therapy treatment plans
- Provide hands-on therapy and adaptive strategies to maximize independence
- Collaborate with nurses, PTs, SLPs, and case managers to coordinate care
- Document patient progress and revise plans of care based on outcomes
- Educate patients and families on home programs and community resources
- Participate in discharge planning to ensure safe transitions
- Maintain professional standards, compliance, and timely communication with the team
Qualifications
- Current Occupational Therapy licensure in New Hampshire (or ability to obtain)
- NBCOT certification preferred or commitment to obtain
- Comfortable working in a skilled rehab setting with adult and geriatric populations
- Strong interpersonal and documentation skills
- Dependable, professional, and able to collaborate effectively with an interdisciplinary team
- Previous outpatient or inpatient rehab experience preferred
Education
- Bachelor's or Master's degree in Occupational Therapy (OT/OTD preferred)
Knowledge & Skills
- Proven knowledge of therapeutic interventions, ADL training, and adaptive equipment
- Skilled in assessment tools, treatment planning, and outcome measurement
- Ability to educate patients and caregivers in an encouraging, practical way
- Familiarity with electronic medical records and timely documentation
- Good clinical reasoning and problem-solving for functional recovery
- Strong teamwork and communication skills with clinical colleagues and families
