Roles & Responsibilities

• Provide patient-centered evaluations and individualized physical therapy treatment plans
• Deliver skilled therapeutic interventions to improve mobility, function, and quality of life
• Track patient progress, document thoroughly in the medical record, and adjust plans as needed
• Collaborate with nursing, rehab aides, and interdisciplinary teams to coordinate care
• Educate patients and families on therapy goals, home programs, and safe mobility strategies
• Participate in team meetings and contribute to continuous quality improvement initiatives

Qualifications

• Current, unrestricted Physical Therapy license in New Hampshire or eligible for licensure
• Doctor of Physical Therapy (DPT) or equivalent degree preferred
• BLS/CPR certification (or willing to obtain upon hire)
• Experience in post-acute, skilled nursing, or outpatient rehab settings is preferred
• Strong documentation skills and comfort using electronic medical records
• Excellent communication and teamwork skills with a patient-first attitude

Education

• Doctor of Physical Therapy (DPT) preferred; Master’s or Bachelor’s in Physical Therapy considered

Knowledge & Skills

• Thorough knowledge of therapeutic exercise, manual therapy, and mobility training
• Skilled at patient assessment, goal-setting, and measurable outcome tracking
• Proficient with documentation standards and rehab-focused EMR systems
• Ability to prioritize caseloads and manage time effectively
• Strong interpersonal skills for working with residents, families, and clinical teams
