Roles & Responsibilities

• Provide compassionate, hands-on nursing care to residents and patients
• Administer medications accurately and document in the chart
• Monitor vital signs and recognize changes in patient condition
• Perform routine treatments and basic wound care as ordered
• Maintain detailed, timely documentation of nursing interventions
• Collaborate with nurses, therapists, and physicians to coordinate care
• Support patient and family education to promote recovery and wellbeing
• Follow infection control and safety protocols to protect patients and staff

Qualifications

• Active LPN license in good standing
• Recent practical nursing experience in long-term care, rehab, or similar setting preferred
• Current Basic Life Support (BLS) certification
• Strong clinical assessment and medication administration skills
• Reliable, team-oriented, and patient-focused attitude

Education

• Graduate of an accredited practical nursing program (LPN/LVN diploma)

Knowledge & Skills

• Medication administration and documentation best practices
• Basic wound care and infection prevention
• Vital sign monitoring and recognizing clinical changes
• Charting and electronic health record familiarity
• Effective communication with patients, families, and interdisciplinary teams
• Time management and prioritization in a fast-paced setting
