Roles & Responsibilities
- Assess resident health status and implement individualized care plans with compassion and accuracy
- Administer medications and treatments safely, following facility protocols and best practices
- Monitor and document changes in condition, communicating promptly with the interdisciplinary team
- Support residents with activities of daily living while promoting dignity and independence
- Provide family education and clear updates to keep loved ones involved and informed
- Participate in quality improvement efforts to enhance resident outcomes and facility standards
Qualifications
- Active Registered Nurse license (RN) — ability to practice in New York is expected
- Current BLS/CPR certification (or willing to obtain prior to start)
- Experience in long term care or skilled nursing settings preferred
- Comfortable administering medications, charting in electronic medical records, and working as part of a multidisciplinary team
- Strong communication skills and a patient-centered approach
Education
- Associate Degree in Nursing (ADN) required; Bachelor of Science in Nursing (BSN) preferred
Knowledge & Skills
- Knowledge of long term care nursing practices and regulatory requirements
- Skill in clinical assessment, medication administration, and wound care basics
- Ability to prioritize tasks in a fast-paced environment and manage multiple resident needs
- Excellent documentation skills and familiarity with electronic charting (EMR)
- Teamwork mindset with strong interpersonal and problem-solving abilities
