Roles & Responsibilities
- Provide hands-on therapeutic treatments and interventions to help patients regain function and independence
- Collaborate closely with Physical Therapists and the interdisciplinary care team to implement individualized care plans
- Document patient progress thoroughly and maintain accurate treatment records
- Coach and educate patients and families on home programs, safe transfers, and activity progression
- Assist with mobility, gait training, transfers, and therapeutic exercise within the SNF setting
- Participate in discharge planning and communicate changes to the care team to optimize outcomes
Qualifications
- PTA skillset (Physical Therapist Assistant) and relevant clinical experience
- 1-3 years of experience working in physical therapy or related patient care (as indicated)
- Experience or comfort working in a Skilled Nursing Facility (SNF) environment
- Strong communication and patient-education abilities
- Current BLS/CPR certification or willingness to obtain if required by facility
Knowledge & Skills
- Hands-on physical therapy techniques and therapeutic exercise
- Patient assessment, mobility assistance, and ADL support
- SNF clinical workflows and interdisciplinary collaboration
- Accurate clinical documentation and use of electronic health records
- Safe transfer and fall-prevention strategies
- Compassionate patient-centered care and family education
