Roles & Responsibilities

• Provide hands-on therapeutic treatments and interventions to help patients regain function and independence
• Collaborate closely with Physical Therapists and the interdisciplinary care team to implement individualized care plans
• Document patient progress thoroughly and maintain accurate treatment records
• Coach and educate patients and families on home programs, safe transfers, and activity progression
• Assist with mobility, gait training, transfers, and therapeutic exercise within the SNF setting
• Participate in discharge planning and communicate changes to the care team to optimize outcomes

Qualifications

• PTA skillset (Physical Therapist Assistant) and relevant clinical experience
• 1-3 years of experience working in physical therapy or related patient care (as indicated)
• Experience or comfort working in a Skilled Nursing Facility (SNF) environment
• Strong communication and patient-education abilities
• Current BLS/CPR certification or willingness to obtain if required by facility

Knowledge & Skills

• Hands-on physical therapy techniques and therapeutic exercise
• Patient assessment, mobility assistance, and ADL support
• SNF clinical workflows and interdisciplinary collaboration
• Accurate clinical documentation and use of electronic health records
• Safe transfer and fall-prevention strategies
• Compassionate patient-centered care and family education

This assignment is based in the St. Louis area (Creve Coeur / Country Villa Wellness & Rehab).
