Roles & Responsibilities
- Provide compassionate, hands-on patient care in a skilled nursing environment
- Administer medications and treatments accurately and document thoroughly
- Monitor residents’ conditions and promptly report changes to the care team
- Collaborate with RNs, therapists, and support staff to create person-centered care plans
- Assist with activities of daily living while promoting independence and dignity
- Maintain clear, timely clinical documentation and uphold facility policies
- Support a positive, team-focused atmosphere that improves resident outcomes
Qualifications
- Active Licensed Practical Nurse (LPN) or Registered Nurse (RN) credential
- Current BLS/CPR certification (or willing to obtain prior to start)
- Comfortable working in a long-term care or skilled nursing setting
- Strong communication skills and a patient-first attitude
- Ability to pass facility onboarding and background checks
Education
- Graduate of an accredited LPN or RN nursing program
- High school diploma or equivalent required
Knowledge & Skills
- Medication administration and basic nursing assessments
- Wound care basics and infection control practices
- Charting and electronic health record familiarity
- Time management and prioritization in a busy clinical setting
- Team collaboration and clear handoff communication
