Roles & Responsibilities

• Provide compassionate, patient-centered nursing care to residents in a skilled nursing setting
• Administer medications and treatments accurately and document per facility policy
• Assess patient conditions, report changes promptly, and participate in care planning
• Collaborate with nurses, therapists and support staff to deliver coordinated care
• Support residents with activities of daily living while promoting dignity and independence
• Maintain clear, timely charting and follow all regulatory and facility documentation standards

Qualifications

• Current Licensed Practical Nurse (LPN) license or Registered Nurse (RN) license — active or eligible in Massachusetts
• Basic Life Support (BLS/CPR) certification required or must obtain upon hire
• Excellent communication and teamwork skills; reliable and professional
• Comfortable working 12-hour day shifts and able to provide hands-on resident care
• Previous experience in long-term care or rehabilitation is a plus but caring attitude and a willingness to learn matter most

Education

• Completion of an accredited LPN program or RN nursing program
• High school diploma or equivalent

Knowledge & Skills

• Strong clinical assessment and basic nursing procedures
• Medication administration and documentation best practices
• Familiarity with electronic health records and charting
• Ability to prioritize tasks, multitask safely, and work well under pressure
• Compassionate bedside manner and effective communication with residents and families
