Roles & Responsibilities
- Evaluate patients and develop individualized physical therapy treatment plans to improve function
- Deliver evidence-based therapy interventions to enhance mobility, strength, balance and independence
- Complete evaluations, progress notes and discharge documentation in compliance with facility and regulatory standards
- Collaborate with interdisciplinary team members to coordinate high-quality patient care
- Educate patients and families on therapy goals, safety and home exercise programs to support recovery
- Ensure adherence to state, federal and facility policies to maintain safe, compliant care
Qualifications
- Active Washington State Physical Therapist license in good standing (required)
- Graduate of an accredited Physical Therapy program
- Skilled nursing facility or post-acute care experience preferred
- Strong clinical judgment with excellent communication and documentation skills
- Ability to work independently and as part of a multidisciplinary team
Education
- Graduate of an accredited Physical Therapy program

- Evidence-based therapeutic interventions for mobility and balance
- Accurate clinical documentation and regulatory compliance
