Roles & Responsibilities

• Provide compassionate direct patient care that promotes dignity and comfort
• Administer medications and treatments safely and document care accurately
• Perform routine assessments and report changes to the care team promptly
• Collaborate with nurses, therapists, and physicians to coordinate patient-centered care
• Support residents with activities of daily living and promote independence
• Maintain accurate charts and comply with facility policies and regulatory standards

Qualifications

• Current Licensed Practical Nurse (LPN) credential — required
• Eligible to work the listed schedule: Sunday, Monday, Wednesday, Friday, Saturday, 3:00 PM - 11:00 PM
• Comfortable working a travel/contract assignment and adapting to new teams and settings
• Willingness to complete facility onboarding, background checks, and required health screenings

Education

• Completion of an accredited Practical Nursing program or equivalent

Knowledge & Skills

• Strong fundamentals in patient care, vital signs, and medication administration
• Skilled at documentation and electronic charting systems
• Good clinical judgment and the ability to escalate concerns appropriately
• Excellent communication and teamwork skills with residents and interdisciplinary staff
• Time management and organization to handle a busy evening shift
