Roles & Responsibilities
- Conduct thorough home health nursing assessments and complete OASIS documentation accurately and on time
- Develop, implement, and update individualized care plans in collaboration with patients, families, and interdisciplinary teams
- Provide skilled nursing interventions and patient education to promote health, safety, and independence at home
- Coordinate care and referrals with physicians, therapists, and community resources to ensure smooth transitions
- Perform regular case management duties including visit planning, documentation, and outcomes tracking
- Communicate proactively with the agency to report changes in patient status and ensure timely follow-up
- Support quality initiatives and compliance with home health regulations and documentation standards
Qualifications
- Registered Nurse (RN) — active and in good standing
- Home health experience with demonstrated case management skills
- OASIS documentation experience required
- Comfortable working as a travel nurse and able to commit to the contract period with no requested time off
- Strong clinical assessment, communication, and documentation abilities
- Reliable transportation and ability to travel locally for patient visits
Knowledge & Skills
- Proven knowledge of OASIS assessment and home health documentation
- Skilled in patient assessment, wound care, medication management, and care planning
- Excellent care coordination and case management skills
- Ability to work independently in patients’ homes and make sound clinical decisions
- Strong interpersonal skills for patient/family education and interdisciplinary collaboration
