Roles & Responsibilities

• Provide compassionate direct patient care under the direction of the RN and facility protocols
• Administer medications and treatments accurately and document per policy
• Monitor patient status, report changes promptly, and participate in care planning
• Perform routine assessments, wound care, and ADL assistance with dignity and respect
• Maintain clear, timely, and professional charting in the electronic health record
• Collaborate with multidisciplinary team members to ensure excellent patient outcomes
• Support patient and family education and offer reassurance during transitions of care

Qualifications

• Current, active LPN license in the state of practice (WV license or compact eligibility as applicable)
• Current BLS (Basic Life Support) certification required
• Recent clinical experience as an LPN in a relevant setting preferred
• Immunizations and health screenings per facility policy (TB, flu, etc.)
• Strong communication skills and ability to work nights as scheduled
• Willingness to be flexible and adapt to facility-specific practices

Education

• Graduate of an accredited LPN/Vocational Nursing program; high school diploma or equivalent required

Knowledge & Skills

• Solid knowledge of medication administration and safety practices
• Patient assessment and basic clinical skills with attention to detail
• Proficiency with electronic health records and accurate documentation
• Infection control, wound care basics, and ADL support
• Ability to prioritize, multitask, and remain calm in fast-paced situations
• Teamwork-minded with strong interpersonal and customer service skills
