Roles & Responsibilities

• Assist the Physical Therapist with implementing individualized treatment plans that improve patient mobility and independence
• Provide hands-on therapy such as gait training, therapeutic exercise, transfers, and balance activities
• Document patient evaluations, treatment progress, and discharge recommendations accurately and on time
• Collaborate with nursing, therapy colleagues, and families to ensure coordinated, patient-centered care
• Educate patients and caregivers on exercises, home programs, and safety strategies to support carryover
• Maintain therapy equipment, ensure infection control practices, and contribute to a safe treatment environment
• Participate in team meetings and discharge planning to support optimal outcomes and transitions

Qualifications

• PTA credential or completion of an accredited PTA program (facility may require state licensure or eligibility)
• Open to all experience levels including new graduates; clinical mentorship opportunities often available
• Comfort working in a Skilled Nursing Facility (SNF) environment
• Strong communication skills and ability to work within an interdisciplinary team
• Current CPR certification as required by facility policies

Education

• Not specified; open to new graduates (completion of an accredited PTA program typically expected)

Knowledge & Skills

• Working knowledge of therapeutic exercise, gait training, transfers, and mobility aids
• Familiarity with SNF workflows and skilled documentation expectations
• Ability to adapt treatments for older adults and medically complex patients
• Skilled in patient and caregiver education to promote safety and functional independence
• Good organizational skills and attention to detail for accurate charting
