Roles & Responsibilities
- Assist the Physical Therapist with implementing individualized treatment plans that improve patient mobility and independence
- Provide hands-on therapy such as gait training, therapeutic exercise, transfers, and balance activities
- Document patient evaluations, treatment progress, and discharge recommendations accurately and on time
- Collaborate with nursing, therapy colleagues, and families to ensure coordinated, patient-centered care
- Educate patients and caregivers on exercises, home programs, and safety strategies to support carryover
- Maintain therapy equipment, ensure infection control practices, and contribute to a safe treatment environment
- Participate in team meetings and discharge planning to support optimal outcomes and transitions
Qualifications
- PTA credential or completion of an accredited PTA program (facility may require state licensure or eligibility)
- Open to all experience levels including new graduates; clinical mentorship opportunities often available
- Comfort working in a Skilled Nursing Facility (SNF) environment
- Strong communication skills and ability to work within an interdisciplinary team
- Current CPR certification as required by facility policies
Education
- Not specified; open to new graduates (completion of an accredited PTA program typically expected)
Knowledge & Skills
- Working knowledge of therapeutic exercise, gait training, transfers, and mobility aids
- Familiarity with SNF workflows and skilled documentation expectations
- Ability to adapt treatments for older adults and medically complex patients
- Skilled in patient and caregiver education to promote safety and functional independence
- Good organizational skills and attention to detail for accurate charting
