Roles & Responsibilities
- Assess patients' physical function and develop personalized treatment plans
- Deliver evidence-based therapy interventions to improve mobility and independence
- Monitor patient progress and adjust plans to meet meaningful goals
- Collaborate with interdisciplinary team members to ensure coordinated care
- Document treatments, outcomes, and discharge plans in a timely and accurate manner
- Educate patients and caregivers on home programs and injury prevention strategies
- Participate in team meetings and contribute to a positive patient-centered culture
Qualifications
- Active or eligible California Physical Therapist license (expected for clinical practice)
- Doctor of Physical Therapy (DPT) preferred; related degrees considered
- Current BLS/CPR certification or willingness to obtain upon hire
- Strong clinical assessment and manual therapy skills
- Excellent communication skills and a collaborative mindset
- Comfortable with documentation and meeting facility standards
Education
- Doctor of Physical Therapy (DPT) preferred; Bachelor’s or Master’s in related field considered
Knowledge & Skills
- Skilled in evaluation, treatment planning, and progress measurement
- Familiarity with common outpatient and inpatient rehab modalities
- Ability to use clinical reasoning to tailor interventions to individual needs
- Proficient in documentation best practices and electronic medical records
- Patient education and caregiver training skills
