Roles & Responsibilities
- Perform comprehensive admission assessments and start-of-care visits to set strong, patient-centered plans
- Deliver high-quality wound care and integumentary assessments with clear documentation
- Administer and manage medications safely, including intramuscular, subcutaneous, and intravenous infusions
- Start and maintain IVs and perform phlebotomy when clinically indicated
- Complete timely and accurate charting in EPIC and OASIS, ensuring documentation meets agency and regulatory standards
- Teach and coach patients and caregivers on disease management, medications, and infection control precautions
- Collaborate with physical therapy, social services, and other team members to coordinate holistic care
- Manage a productive caseload (may visit up to six patients per day) while maintaining quality and patient safety
Qualifications
- Active RN license in Oregon (pending licenses are accepted for this assignment)
- Minimum 1 year of skilled home health nursing experience required
- OASIS experience is required — please highlight this on your application
- BCLS certification required
- Current COVID-19 primary vaccination series plus one booster required by facility
- Comfort with charting in EPIC and OASIS; EPIC experience is preferred
- Strong IV/infusion and phlebotomy skills
- Reliable own transportation for patient visits across the service area
- First-time travel nurses are not accepted for this opening
Education
- Graduate of an accredited nursing program (ADN or BSN) and active RN licensure or pending RN licensure accepted
Knowledge & Skills
- Proficient in OASIS documentation and start-of-care/admission assessments
- Skilled in medication administration and safe infusion practices
- Wound care assessment and treatment skills, including dressing selection and documentation
- IV insertion and maintenance plus phlebotomy competency
- Familiarity with infection control precautions in the home setting
- Solid clinical judgment in adult and geriatric home health populations
- Comfortable using EPIC for charting (preferred) and meeting productivity expectations
