Roles & Responsibilities

• Assess patient function and mobility in home environments and create individualized treatment plans
• Deliver skilled home health physical therapy interventions focused on orthopedics and neurologic rehabilitation
• Educate patients and families on home exercise programs and safe body mechanics to support recovery
• Document accurately and timely using EPIC and OASIS to maintain continuity of care
• Coordinate care with interdisciplinary teams to optimize patient outcomes
• Manage daily caseload efficiently — visits may be up to 6 patients per day while maintaining quality care
• Maintain professional standards including BCLS and adherence to facility vaccination requirements

Qualifications

• Current Physical Therapist license (state license required; pending licenses accepted — confirm with recruiter)
• Minimum 1 year of skilled home health experience required
• BCLS (Basic Cardiac Life Support) required
• Experience with EPIC and OASIS charting strongly preferred — call out if you do not have OASIS experience
• Reliable transportation for home visits and ability to travel within occasional 50+ mile radius
• No first-time travelers accepted for this assignment

Knowledge & Skills

• Assessment and treatment planning in home health settings
• Home Exercise Program design and patient/family education
• Orthopedics: total hip/knee replacements, post-op shoulder and spinal surgery care
• Neurologic rehabilitation: post-stroke, Parkinson’s, TBI, spinal cord injury
• Comfort charting in EPIC and completing OASIS documentation
• Strong time management to handle up to 6 visits/day and productivity expectations
