Roles & Responsibilities

• Perform thorough patient assessments and document OASIS-compliant evaluations
• Develop and implement individualized home exercise programs that empower patients
• Deliver hands-on skilled physical therapy focusing on orthopedics and neurologic rehabilitation
• Educate patients and families to maximize safety, function, and independence at home
• Chart accurately and timely in EPIC (remote client) and Rover phone app
• Manage your caseload efficiently — may visit up to 6 patients/day while meeting productivity standards
• Collaborate with interdisciplinary team members to coordinate patient-centered care

Qualifications

• Current PT license for Oregon (pending licenses accepted — confirm with recruiter)
• Minimum 1 year of skilled home health experience required
• BCLS (BLS) certification required
• Comfortable using EPIC, Rover and completing OASIS documentation
• Reliable transportation and willingness to travel within an occasional 60-mile radius (60–90 minutes total drive time some days)
• Facility requires primary COVID-19 vaccine series plus one booster (facility-specific requirement)

Knowledge & Skills

• Skilled in patient assessment and treatment planning for home health populations
• Strong foundation in body mechanics and therapeutic exercise prescription
• Experience with ortho diagnoses — total hip/knee replacement, post-op care, spinal conditions
• Neurologic rehab experience — stroke, spinal cord injury, Parkinson’s, MS, TBI
• Excellent patient and family education skills to promote home safety and independence
• Charting proficiency with OASIS and electronic health records (EPIC, Rover)
