Roles & Responsibilities

• Deliver high-quality, compassionate direct patient care as an LPN
• Administer medications and treatments safely and accurately
• Monitor patient status and report changes promptly to the care team
• Collaborate with RNs, providers, and interdisciplinary staff to support care plans
• Document assessments, interventions, and patient responses clearly and timely
• Provide patient and family education with empathy and clarity
• Support discharge planning and coordinate transitions of care

Qualifications

• Current Licensed Practical Nurse (LPN) licensure - active and in good standing
• Completion of an accredited LPN/vocational nursing program
• Basic Life Support (BLS) / CPR certification (or willingness to obtain)
• Current immunizations and ability to meet facility health requirements
• Strong clinical assessment, medication administration, and documentation skills
• Excellent communication, teamwork, and a patient-centered attitude

Education

• Graduation from an accredited LPN/Vocational Nursing program

Knowledge & Skills

• Solid knowledge of medication administration, IV basics as applicable, and wound care
• Familiarity with electronic health records and accurate charting practices
• Ability to prioritize tasks, manage time during long shifts, and handle emergencies
• Strong interpersonal skills for working with patients, families, and staff
• Basic understanding of infection control, safety protocols, and regulatory standards
