Roles & Responsibilities

• Provide compassionate bedside care and support to patients at Hopkins Center
• Administer medications and treatments safely and according to plan
• Monitor and document vital signs and patient status accurately
• Collaborate with nurses, therapists, and interdisciplinary team members to deliver coordinated care
• Perform skilled nursing tasks such as wound care and basic assessments
• Educate patients and families on care plans and discharge instructions
• Maintain accurate, timely documentation in the patient record
• Support patient mobility, activities of daily living, and promote dignity

Qualifications

• Active LPN license (Licensed Practical Nurse) — must hold or obtain appropriate state licensure
• Current BLS/CPR certification (or ability to obtain prior to start)
• Comfortable working in long-term care / rehabilitation settings (experience preferred)
• Strong communication skills and a patient-centered approach
• Ability to pass facility-required screenings and background checks

Education

• Completed accredited practical nursing program (LPN/LVN diploma or certificate)

Knowledge & Skills

• Medication administration and basic clinical assessments
• Skilled wound care and infection prevention practices
• Patient safety, fall prevention, and mobility assistance
• Accurate charting and documentation skills
• Teamwork, delegation, and effective handoff communication
• Time management and prioritization during busy shifts
