Roles & Responsibilities

• Provide compassionate, hands-on nursing care to residents at the Hopkins Center
• Administer medications and treatments accurately and document thoroughly
• Perform vital signs and patient assessments, reporting changes to the care team
• Collaborate with RNs, therapists, and interdisciplinary team members for great outcomes
• Educate residents and families about care plans and daily routines
• Maintain accurate EMR/charting and complete shift-to-shift handoffs
• Follow infection control, safety protocols, and facility policies to protect patients

Qualifications

• Active LPN license in Pennsylvania (required)
• Graduation from an accredited practical nursing program
• Current CPR/BLS certification (or willingness to obtain prior to start)
• Previous experience in long-term care, skilled nursing, or similar preferred
• Ability to pass background checks and meet facility onboarding requirements
• Strong communication skills and a team-first attitude

Education

• Graduate of an accredited Practical Nursing (LPN) program
• Diploma or certificate in Practical Nursing

Knowledge & Skills

• Medication administration and documentation best practices
• Basic wound care, ADL assistance, and resident assessments
• Familiarity with electronic medical records and charting
• Good clinical judgment and ability to escalate changes promptly
• Time management and prioritization during busy evening shifts
• Compassionate bedside manner and resident-centered care approach
