Roles & Responsibilities

• Provide compassionate, hands-on nursing care to residents in accordance with care plans
• Administer medications and treatments safely and accurately
• Perform timely patient assessments and escalate changes in condition
• Document care clearly and thoroughly in the patient record
• Collaborate with the interdisciplinary team to deliver coordinated care
• Support patient comfort, dignity and independence during each shift
• Assist with admissions, discharges and transfers as needed

Qualifications

• Current Licensed Practical Nurse (LPN) license (Pennsylvania or ability to obtain state licensure prior to start)
• Basic Life Support (BLS/CPR) certification required or obtain upon hire
• Comfortable with medication administration, wound care basics, and routine assessments
• Strong communication skills and a team-oriented mindset
• Previous experience in long-term care, skilled nursing, or similar clinical setting preferred

Education

• Graduate of an accredited Practical Nursing program (diploma or certificate)

Knowledge & Skills

• Knowledge of LPN scope of practice and state regulations
• Skill in medication administration and documentation
• Ability to conduct focused patient assessments and recognize changes in condition
• Familiarity with electronic health records and charting best practices
• Good infection control practices and safety awareness
