Roles & Responsibilities

• Provide compassionate, hands-on nursing care to residents in the Hopkins Center
• Administer medications and treatments accurately and safely
• Perform timely assessments and share relevant observations with the care team
• Document care thoroughly in the facility electronic health record
• Collaborate with nurses, therapists and physicians to deliver coordinated care
• Support resident comfort and dignity while promoting independence
• Educate residents and families on care plans and best practices

Qualifications

• Active Licensed Practical Nurse (LPN) license — Pennsylvania license preferred
• Current BLS (Basic Life Support) certification required or obtained upon start
• Previous LPN experience in long-term care, subacute or similar settings preferred
• Up-to-date immunizations and any facility-required health screenings
• Reliable, professional, and a team player who communicates clearly

Education

• Completion of an accredited Practical Nursing program (LPN diploma or certificate)

Knowledge & Skills

• Strong clinical skills in medication administration and wound care
• Comfortable performing resident assessments and care planning
• Proficient with documentation and charting in an electronic health record
• Excellent communication and interpersonal skills with residents and families
• Ability to prioritize, multitask and remain calm in a fast-paced setting
• Team-oriented mindset and willingness to support colleagues
