Roles & Responsibilities

• Evaluate patients and develop individualized treatment plans that promote independence and functional gains
• Provide skilled therapeutic interventions including therapeutic exercise, gait training, balance and mobility training
• Monitor patient progress, adjust interventions, and document outcomes in a timely, professional manner
• Collaborate with nursing, OT, SLP and case management to coordinate comprehensive care
• Educate patients and families on home programs, fall prevention, and safe mobility techniques
• Participate in team meetings and discharge planning to ensure smooth transitions of care
• Adhere to facility policies, payer documentation requirements, and professional standards

Qualifications

• Licensed Physical Therapist or eligible for PT licensure in Pennsylvania (PA license required prior to start)
• Graduate of an accredited Physical Therapy program (DPT or Master’s level acceptable)
• Current BLS/CPR certification or willingness to obtain prior to start
• Experience in skilled nursing, inpatient rehab, or post-acute care is preferred
• Clean background check and current immunizations as required by facility
• Strong documentation skills and familiarity with outpatient and inpatient therapy workflows

Education

• Doctor of Physical Therapy (DPT) or Master of Physical Therapy from an accredited program; new grads eligible if license obtained

Knowledge & Skills

• Skilled in assessment of mobility, balance, gait and functional transfer training
• Proficient in therapeutic exercise prescription and progressive care planning
• Comfortable with assistive device training and fall prevention strategies
• Excellent verbal and written communication with patients, families, and team members
• Able to prioritize caseload and manage timely documentation
• Familiarity with Medicare and documentation best practices preferred
