Roles & Responsibilities
- Evaluate patients and develop individualized treatment plans that promote independence and functional gains
- Provide skilled therapeutic interventions including therapeutic exercise, gait training, balance and mobility training
- Monitor patient progress, adjust interventions, and document outcomes in a timely, professional manner
- Collaborate with nursing, OT, SLP and case management to coordinate comprehensive care
- Educate patients and families on home programs, fall prevention, and safe mobility techniques
- Participate in team meetings and discharge planning to ensure smooth transitions of care
- Adhere to facility policies, payer documentation requirements, and professional standards
Qualifications
- Licensed Physical Therapist or eligible for PT licensure in Pennsylvania (PA license required prior to start)
- Graduate of an accredited Physical Therapy program (DPT or Master’s level acceptable)
- Current BLS/CPR certification or willingness to obtain prior to start
- Experience in skilled nursing, inpatient rehab, or post-acute care is preferred
- Clean background check and current immunizations as required by facility
- Strong documentation skills and familiarity with outpatient and inpatient therapy workflows
Education
- Doctor of Physical Therapy (DPT) or Master of Physical Therapy from an accredited program; new grads eligible if license obtained
Knowledge & Skills
- Skilled in assessment of mobility, balance, gait and functional transfer training
- Proficient in therapeutic exercise prescription and progressive care planning
- Comfortable with assistive device training and fall prevention strategies
- Excellent verbal and written communication with patients, families, and team members
- Able to prioritize caseload and manage timely documentation
- Familiarity with Medicare and documentation best practices preferred
