Roles & Responsibilities

• Deliver compassionate daily nursing care to residents and patients
• Administer medications and treatments accurately and on schedule
• Assess, document, and communicate changes in patient condition
• Collaborate with the interdisciplinary team to coordinate care plans
• Provide patient and family education with clarity and empathy
• Maintain accurate electronic health records and comply with facility policy
• Support a safe, clean environment through infection control and safety practices

Qualifications

• Active LPN license in Pennsylvania required
• Current BLS/CPR certification required or must obtain upon hire
• Experience in long-term care or skilled nursing preferred
• Strong communication skills and a patient-centered approach
• Ability to pass required background checks and immunization verification

Education

• Completion of an accredited practical nursing program and state licensure

Knowledge & Skills

• Medication administration and documentation
• Basic wound care and ADL support
• Familiarity with electronic health record charting
• Good clinical assessment and triage skills
• Teamwork, time management, and clear communication
