Roles & Responsibilities
- Provide safe, compassionate nursing care to residents in the rehab center
- Administer medications and treatments accurately and document them thoroughly
- Perform skilled assessments and communicate changes in condition to the care team
- Collaborate with nurses, therapists, and physicians to implement care plans
- Support resident mobility, hygiene, and activities of daily living with dignity
- Educate residents and families about care plans, medications, and safety
- Participate in shift handoffs and accurate charting to ensure continuity of care
Qualifications
- Active Licensed Practical Nurse (LPN) license (state-required)
- CPR/BLS certification or willingness to obtain promptly
- Local candidates preferred (LOCAL ONLY) — reliable commuting to Laconia Rehab Ctr
- Strong communication skills and ability to work as part of a multidisciplinary team
- Comfortable with medication administration, documentation, and basic assessments
Education
- Completion of an accredited Practical Nursing program (or equivalent)
- High school diploma or GED required
Knowledge & Skills
- Patient assessment and monitoring
- Medication administration and documentation best practices
- Experience with rehab or long-term care procedures preferred
- Infection control and safe patient handling principles
- Compassionate patient-centered care and family communication
