Roles & Responsibilities
- Provide compassionate bedside care to residents, focusing on safety and dignity
- Administer medications and treatments accurately and on schedule
- Monitor vital signs and changes in condition, escalating concerns to the RN or provider
- Document resident status, interventions, and outcomes clearly and promptly
- Assist with activities of daily living while promoting resident independence
- Collaborate with the care team to implement individualized care plans
- Support a positive, respectful environment for residents and families
Qualifications
- Current Licensed Practical Nurse (LPN) license valid in Pennsylvania
- Active, unrestricted nursing license and ability to meet facility credentialing requirements
- Previous LPN experience in long-term care, skilled nursing, or similar setting preferred
- Comfort with night shift responsibilities and independent clinical decision-making
- Strong communication skills and a patient-centered attitude
Education
- Graduate of an accredited Practical Nursing program or equivalent vocational nursing certificate
Knowledge & Skills
- Proficient in medication administration and basic clinical procedures
- Skilled at vital signs monitoring, wound observation, and documentation
- Ability to follow care plans and report changes in resident condition
- Good time management and prioritization during busy shifts
- Team-oriented with strong interpersonal and communication skills
