Roles & Responsibilities

• Evaluate patients' functional abilities and develop individualized treatment plans
• Deliver hands-on therapy and evidence-based interventions to improve mobility and independence
• Collaborate with rehab team members to coordinate comprehensive patient care
• Document patient progress accurately and timely in the facility’s records
• Educate patients and families on home programs and strategies for long-term recovery
• Participate in team meetings and contribute to continuous improvement of clinical practices

Qualifications

• Licensed Physical Therapist in Wisconsin or eligible for Wisconsin licensure
• Doctor of Physical Therapy (DPT) preferred, or equivalent degree with required state licensure
• BLS/CPR certification (or willingness to obtain upon hire)
• Experience in a rehabilitation or outpatient setting preferred
• Strong communication skills and ability to work collaboratively with multidisciplinary teams
• Reliable, professional, and patient-centered approach to care

Education

• Doctor of Physical Therapy (DPT) preferred; Bachelor's or Master's in a related field acceptable with appropriate licensure

Knowledge & Skills

• Skilled in therapeutic exercise, manual therapy, gait training, and functional mobility training
• Familiarity with common rehab diagnoses and appropriate interventions
• Comfortable with documenting in electronic medical records (EMR) systems
• Ability to perform safe patient handling and use assistive devices
• Strong clinical reasoning, time management, and patient education skills
